Library Card Application

Leave This Blank:

A library card is free to any permanent resident of the City of DeSoto. To obtain a card, a person must show a valid driver's license with a current local address, or other proof of residency such as a current utility bill, current voter's registration card, or a signed lease or purchase agreement on a residence.

Children under 16 applying for library cards must be accompanied by a parent or guardian with the acceptable identification. Children must be at least 6 years old to have a library card.

The initial card for City of DeSoto residents is free of charge. Lost cards may be replaced for a fee of $3.00.

Non-resident Library cards are issued to those people who live outside the City limits of DeSoto and are available for a nominal charge. Non-resident cards are valid for one year from the purchase date.

If you wish, you may fill out the form below to submit the information needed to issue a library card to you. You will still need to come in to the library to show your picture identification and proof of residency before being issued the card, but submitting the rest of the information before will speed up the process.

If you have any other questions about getting a library card - please call the Library at 972-230-9656 during regular business hours.

(required if card is for a child)
